Frozen Date As of

Frozen Date As of  indicates the static date on which membership counts of Per Capita invoices are based.  This is always the 15^th of the prior month.

This is the date that the report is accessed.  This date will change on subsequent viewing

Source

The source is where the information on the Frozen Membership Roster is derived.  The source could be one of the following:  Membership Suite, Membership Lite, Paper Suite, NYSUT, Affiliate Transfer Files or Transition.

Last Update

Last Update shows membership counts as of a specified time frame. 

Local Name & Number 

This is the name of a local and its associated local number.

Chapter Name & Number

The name of a chapter within a local and the associated chapter number.  The chapter’s name may or may not be the same as that of the local. 

Membership Suite users:

The chapter and/or chapters within a local are listed in increasing numerical order.  For example, if the local number is 1234, then the first chapter listed will be 1234A, followed by chapter 1234B, and so on.

Name of Member

The members are listed in ascending (a…z) alphabetical order by last names and then by first names.

Address

Only the first line of the member’s mailing address is listed.

National Dues

Both the numerical code and a description of the national dues paid are listed.

AD&D

The Accidental Death and Dismemberment (AD&D) Insurance coverage must show either a “Y” for Yes or “N” for No. Only “Yes” are counted on the Per Capita Invoice.

LIAB

 The Liability (LIAB) Insurance coverage must show either a “Y” for Yes or “N” for No.

State Dues
Only applies to the following states: CA, CT, DE, GA, IA, MD, MA, NJ, NC, OH, TN, WI

Both the numerical code and a description of the State Dues paid are listed, if applicable.

Summary

 A summary report is listed for each chapter within a local.

Grand Total

A final summary report incorporating the summaries of all the chapters within a local.
